We want to ensure that we provide excellent, individualised end of life and bereavement care for every patient and every family, every time. The Swan Model allows us to be entirely patient and family focused, meeting the unique needs of each individual and their loved ones. It is instigated at point of recognition of dying and is used to support care throughout end of life, into bereavement and beyond. 

Our ethos is to empowering teams, and giving them permission to care and to break the rules that don’t exist where necessary to support individuals and families and any donations ensure that our teams have the resources that they need to uniquely support individual needs.

This unique service goes above and beyond for patients and their families and many of the special touches that we offer are thanks to charitable donations.
